Landlord-Tenant Court Forms
Landlords can use the following forms to put their tenants on notice of health hazards, illegal drug activity, lease termination or non-payment of rent. These forms have been approved by the Supreme Court Administrative Office (SCAO) for use in Michigan District Courts. If tenants fail to comply after receiving one of the notices below, contact Paul J. Raine, PLLC to determine the appropriate course of action.

| 7-day notice | Use this form if you are a landlord, your tenant has not paid rent, and you want to give the tenant notice to either pay rent in 7 days or move out. | |
| Health Hazard | Use this form if you are a landlord, your tenant has created a health hazard on the property or has damaged the property, and you want to give the tenant notice to either correct the problem in 7 days or move out. | |
| 30-day notice | Use this form if you are a landlord and you want to give your tenant notice to move out because the tenancy has ended or for some other reason. | |
| | 30-day notice for Mobile Home Parks | Use this form if you are a landlord and you want to give your tenant notice to move the mobile home out of the mobile home park for just cause. | |
| 24-hour notice for illegal drug activity | Use this form if you are a landlord and your tenant willfully or negligenty caused unlawful drug activity at the leased premises. | |
